Head left toward the large Lighthouse.

To find the relic from Kudaai’s clue in Cult of the Lamb, head to Smuggler’s Sanctuary.

Step-by-Step Solution

Travel to Smuggler’s Sanctuary from your cult’s teleportation circle.
Walk to the left / upper-left of the area, near Plimbo.
Locate the giant pack-carrying fish.
Walk directly up to the fish until an interaction prompt appears, then choose “Tickle the fish”.
The fish will spit out the relic: Kudaai’s Fingernail.
